As a contracted provider with Healthy Blue, Angels Dome offers a range of home- and community-based services designed to support members in maintaining independence and safety at home.
Covered Services Provided by Angels Dome:

• Personal Care Services (PCS) – assistance with ADLs (bathing, grooming, mobility, toileting, feeding).

• In-Home Aide (IHA) – non-medical support for daily living and safety monitoring.

• Respite Care – temporary caregiver relief in the home.

• Community Inclusion & Support.

• Care Coordination support (working with case managers to maintain authorized service levels).

Angels Dome partners with Carolina Complete Health to provide dependable personal care and support services to Medicaid beneficiaries across eligible counties.
Covered Services Provided:

• Personal Care Services (PCS)

• In-Home Aide Services

• Respite Services

• Community-Based Support Services

• Long-Term Services & Supports (LTSS) assistance for eligible members.

As a Tailored Plan, Trillium covers individuals with behavioral health needs, I/DD, and TBI.
Angels Dome provides the following under Trillium:

• Personal Care Services

• In-Home Aide Services

• Respite (In-home)

• Tailored Plan Physical Health Services (from your PH contract)

• Collaboration with care managers for individuals with I/DD or TBI.

Angels Dome is contracted with Alliance Health to deliver physical health and home-based support services.
Services Provided Under Alliance:

• Personal Care Services (PCS)

• In-Home Aide

• Respite Care

• All Physical Health Codes approved under our contract.

• Care coordination with Alliance case managers
